For all policy contracted by companies
In case of accident or natural disaster if payment is more than $10, 000. 00 USD they must complete the following form: CUSTOMER HISTORY IDENTIFICATION (in Spanish) and collect the information described in DOCUMENTATION NOTICE ART. 140 (in Spanish).
Once the form is completed and gathered the necessary information must be sent to the Area of operations.
